Customer Information Form

Customer Information Form must be filled out if you meet ONE or more of the following requirements:

  • You have never done business with Diverse Group brands 

  • You are a new owner taking over a business that was already buying from Diverse Group

  • You are an existing customer with Diverse Group but you have not done business with us in over 2 years (these accounts have been marked with an INACTIVE status with Diverse Group)

  • You would like to change payment terms

CREDIT CARD AUTHORIZATION FORM

The Credit Card Authorization Form must be filled out if you meet ONE or more of the following requirements:

  • You are a new business that has been established within the last 2 years

  • You would prefer to pay for orders by credit card

  • You are a new owner taking over a business that was already buying from Railtown Apparel Group

  • You would like to change the credit card that you are using

  • You have a credit card that has expired
